STEPS TO TAKE IF YOU HAVE AN ACCIDENT

All EOU employees, faculty, staff and students, are covered by workman's compensation insurance if they are hurt or contract a work-related illness in the course of their employment at EOU.

If you, or one of your employees, are hurt in the course of your work at EOU notify your supervisor immediately. It is important to notify your supervisor even if the injury appears to be minor and does not require a doctor's care. If the injury does not require treatment by a doctor, an accident form is filled out. If the injury requires treatment by a physician, a Workers' Compensation Claim Form 801 must be completed. Building managers, supervisors, Safety/Security, and Human Resources have claim forms and instructions on the steps necessary to submit a claim form 801. Form 801 is also available on the Workers' Compensation Division's Web site:

It is critical that a claim form be filled out as soon as possible after an injury occurs that requires medical treatment. Failure to initiate the claim form 801 in a timely manner may result in denial of benefits. Failure to provide clear and complete information on the claim form may result in a delay of benefits or cause incorrect benefits to be paid.
