PROPOSAL FORM

Proposals should be no longer than two typewritten pages and should include:

1. The program(s) supported by the proposal. Multidisciplinary proposals are encouraged.

2. Name(s) of individuals responsible for executing the proposal.

3. Equipment to be purchased and/or maintained.

a) Description of equipment or maintenance

b) Complete budget. In case of a purchase, identify source of funding for lifetime maintenance of the equipment or include such funding in the proposal.

4. Statement of benefits which further the program(s)

a) Specify discipline(s) and course(s)

b) Specify pedagogical improvements

c) Benefits to other students of faculty
